Family Support Specialist
The Family Support Specialist provides direct assistance including information, referrals, and problem-solving consultation for parents of children with disabilities and special health care needs. Provides training on a variety of family support topics. This is a full-time, grant-funded position supervised by the Director of Family Support.
Responsibilities:
• Front line family support and intake. Assess the needs of the family and identify appropriate services and support. Document requests, types of issues, methods of support, and conduct follow-up. Enter data on family history and demographic characteristics into the database.
• Provide concise factual descriptions of eligibility for state programs and information to help the parent understand the child’s disability. Assist parents with problem-resolution and systems navigation (using state systems and services.)
• Assist with activities in Parent to Parent mentoring and follow-up, Physician Training, parent volunteer program, outreach strategies and events for families of diverse language and culture, and underserved populations.
• Organize and maintain current and reliable community resource information, maintain and update information and contacts on community services. Provide input from parents on their needs and satisfaction with state services for CYSHCN
• Assist with identifying and producing family-friendly, current information for electronic and print distribution (website, newsletter, list serves, and social media) to organize and update information on healthcare reform, support groups, state agency services, community programs, NICU outreach, and transition needs of young adults
• Present training on family support topics and produce evaluations of training, conduct phone surveys, collect monthly impact data, data entry, and reports
Required Qualifications:
• Parent of a child or youth with a disability
• Exceptional customer service and communication skills
• Knowledge of state agency programs and services for families of children with disabilities. Ability to guide families through eligibility and problem resolution options (system navigation).
• Strong presentation skills and ability to present workshops on a variety of topics for parents and professionals.
• Understand and observe compliance with confidentiality requirements.
• Maintain standards of professional conduct with staff, volunteers, health and social service professionals, parent groups, and key community and agency stakeholders.
• Able to perform data entry, conduct surveys, complete evaluations, prepare reports
• Proficiency with office technology. Effective time management in a variety of tasks.
• Available for occasional travel, evening or weekend hours.
• Preferred: Bachelor’s degree social work, health professions, or related discipline.
• Preferred: Bi-lingual in Spanish.
